Ask any diesel tech which high-pressure fuel pump they'd rather have under the hood and you'll get the same answer without hesitation: the Bosch CP3. What these pumps do
Both the CP3 and CP4 are high-pressure common-rail supply pumps. They take low-pressure fuel from the tank and lift pump, then pressurize it to the tens of thousands of PSI the injectors and rail need to atomize fuel for a modern emissions-compliant burn. Rail pressure is the whole ballgame on a common-rail diesel. When the pump lets go, the truck stops.
The difference is in how they build that pressure, and that difference is why one is legendary and one is feared.
The CP3: a radial-piston tank
The Bosch CP3 uses three radial pistons driven off an eccentric on the pump camshaft. The critical detail is that the pistons ride on a rotating eccentric through a shoe, so the contact surfaces are always moving relative to each other and always swimming in fuel. That geometry is extremely tolerant of marginal fuel, brief lubrication interruptions, and general abuse.

The CP3's reputation is earned. Guys make big power on stock CP3s, run them hundreds of thousands of miles, and the pump is rarely the thing that fails. When it does, it usually just gives up pressure quietly rather than sending shrapnel downstream.
The CP4: two pistons and a cam-follower problem
The Bosch CP4 (specifically the CP4.2 on light-duty pickups) was designed to make even higher pressure to meet tightening emissions standards, and to do it with fewer moving parts. It uses two pistons driven by a camshaft with roller-follower buckets. Each piston sits on a follower that's supposed to roll across the cam lobe.
Here's the failure mode. That roller follower has to stay indexed and keep rotating. If it loses its film of fuel for even a moment, or if the roller stops rotating and starts sliding, the follower flat-spots and the cam lobe starts chewing into it. Once metal starts coming off, the pump grinds itself apart internally and pumps its own debris straight into the rail, the lines, and every injector. That's why a CP4 failure isn't a pump replacement, it's a full fuel-system replacement: pump, rail, lines, injectors, and a flush of the tank and everything in between. It's routinely a five-figure job.

Why the CP4 fails and the CP3 doesn't
Three things stack up against the CP4:
Fuel lubricity. US ultra-low-sulfur diesel is drier than the European fuel the CP4 was validated on. The pump relies on diesel itself for lubrication of that cam-and-follower interface, and our fuel gives it less to work with.
Dry starts and air. Any air ingestion, a run-dry from an empty tank, or an owner who accidentally puts gasoline in the tank can wipe the lubricating film off the follower in seconds. The CP3's swimming-shoe geometry shrugs this off. The CP4's point-contact follower does not.
Failure consequences. Even if the CP4 failed at the same rate as a CP3, the outcome is far worse because it self-contaminates the entire system. A CP3 tends to fail gracefully. A CP4 grenades.
None of this means every CP4 truck is a ticking bomb. Plenty run to high mileage with no issue. But the risk profile is real, and it's why the aftermarket built an entire industry around addressing it.
What you can actually do about it

Upgrade filtration and add lubricity. The single best thing you can do is give the pump the cleanest, best-lubricated fuel possible. A high-quality fuel filter, changed on a tight interval, plus an appropriate fuel additive with lubricity improver, directly targets the CP4's weakness. Aftermarket disaster-prevention and secondary filtration kits exist specifically for this.
Keep the tank full and vent-free. Never run the tank to empty on a CP4 truck. Air is the enemy.
Consider the failure math on used trucks. If you're shopping a 2011–2019 6.7 Power Stroke or an LML Duramax, budget for the risk. A CP3 conversion is a known path on some platforms, but it's a fueling decision to make with a reputable shop, and it must be done properly.

Can I convert a CP4 truck to a CP3? On several platforms, yes, and it's a common upgrade because the CP3 is more robust. It's a legal fueling change, not an emissions modification, but have a competent shop do it and use a proven kit. It's not a bolt-on for a driveway on a lunch break.
Will a lift pump save my CP4? It helps. Consistent low-pressure supply and better filtration reduce the odds of air ingestion and dry contact. It's not a guarantee, but it moves the odds in your favor.
