Check the front of the cooling stack for packed bugs and debris that block air between the radiator and the charge-air cooler. Confirm the coolant level and type, and pressure test the system and the surge-tank cap so a small leak or weak cap is not quietly stealing margin. Note whether the temperature climb lines up with an active regen, which is normal added heat.
Then verify flow and fan behavior. Make sure the fan clutch engages and pulls hard when the engine is hot, and watch coolant temperature on a scan tool during a loaded climb to judge how quickly it recovers afterward. A water pump moving less coolant, a lazy thermostat, or an aged radiator will all allow heat to build under sustained load. It also helps to know your baseline: log the normal coolant temperature on a level highway run with no trailer, then compare it to a loaded grade, because a truck that idles and cruises cool but spikes only under a heavy pull is pointing you straight at a marginal cooling stack or flow issue rather than a sensor or gauge fault.
